LPN Residential Services
Provenance Recruitment & Staffing is currently seeking a Licensed Practical Nurse (LPN) to support residential programs within a local Board of Disabilities and Special Needs. This role provides nursing services across multiple programs and requires flexibility, strong clinical skills, and the ability to collaborate with staff, physicians, and families.
This is a full-time, direct hire role with consistent daytime hours.
Job Summary:
The Licensed Practical Nurse will provide agency-wide nursing services including medication oversight, health-related training, wound care, compliance monitoring, and patient care assessments. This role ensures that all programs remain in compliance with nursing standards, medication policies, and quality assurance requirements. The LPN will work closely with program directors, physicians, and direct care staff to support consumer health and wellness.
Key Responsibilities:
- Review medication administration records and ensure compliance with medication error reporting.
- Monitor medication storage and procedures across facilities.
- Review and update patient care plans regularly.
- Train direct care staff on health-related topics.
- Educate consumers on quality assurance and health issues.
- Review doctor’s orders to ensure compliance with policies.
- Pull and return expired medications to pharmacies.
- Conduct nursing assessments of consumers and make recommendations.
- Review and recommend updates to medical, pharmacy, and nursing policies.
- Contact doctors for orders, appointments, and treatments.
- Ensure compliance with psychotropic drug reporting requirements.
- Conduct tardive dyskinesia assessments every 6 months.
- Maintain current LPN license in good standing.
- Participate in on-call rotation with nursing team.
- Review medical evaluations and follow-up reports.
Requirements:
- High school diploma or GED, plus vocational/technical nursing education.
- Current South Carolina Board of Nursing LPN license (must be maintained).
- Minimum of 1 year of experience as an LPN.
- Valid South Carolina driver’s license.
- Ability to train staff and educate consumers on health topics.
- Ability to walk, stand, sit, bend, lift, and perform physical nursing duties (up to 50 lbs).
Work Location: In person – Liberty, SC
Job Type: Full-time
Schedule: Monday–Thursday, 8:00 AM – 5:00 PM; Friday 8:00 AM – 3:00 PM (38 hours per week)
Pay: $23 – $25/hr
